When you are in Windows Explorer with medium icons, is the artwork showing as the file icon? If you open WMP is the artwork showing here? If the artwork is showing there, have you synced using WMP? Like I said before, MP3Tag always work perfect for me. Changing the ID3 version to an older version was a shot in the dark.
